We truly appreciate all of our mission teams and volunteers. Here are many of the documents that we need to prepare for your visit. Return With Deposit
Payment Summary	Please return the Payment Summary with your deposit payment so that me may reserve space for your group!
Group Roster (pdf) or (Excel)	Return with deposit payment. Please include shirt sizes for all of your participants so that everybody can get one!

Return 14 Days Prior to Visit
Medical Information	Medical Information for each participant.
Group Leader Waiver	Liability Waiver for Group Leaders.
Individual Waiver	Liability Waiver for Individuals.
Adult Skills List & Youth Skills List	Please return one for each member of your group. These will help us to plan work opportunities that will best use your skills. Everyone has skills that we can use!
Adult Photo Permission	Please return one for each Adult in your group (use form below for youth).
Youth Photo Permission	Please return one for each person under age 18 in your group.

Please Bring With You
Interview	Please complete and return for each member of your group. We may use this information in newspaper articles and other media.
Volunteer Time Sheet (example)	Please print enough copies of the Volunteer Time Sheet for each member of your group and bring them along.
Caving Waiver	This document is optional. However, it is required for everyone who would like to go caving. Caving is an optional activity and only available when our caving guides are here. The tentative caving dates for 2008 are: July 10, 13-14, 24, 27, August 1. For more information feel free to email our caving guides.
